Ganglion or cyst frequently develops around knee joint. Intraneural ganglion, which is rare disease, affects in the peroneal nerve in the lower limb or the ulnar nerve in the upper extremity. They may cause focal neurologic deficits. We have experienced a rare case of huge ganglion cyst, which was mistaken for deep vein thrombosis or edema, involving tibial and sural nerve. A 50-year-old female, who had suffered from rheumatic arthritis, complained of foot pain and diffuse swelling of right lower limb. Electrodiagnostic study was suggestive of right tibial and sural neuropathy, which could not be localized. Magnetic resonance image showed that huge multilocular cyst was located from the popliteal fossa to the ankle. After resection of the huge cysts, histologic findings showed that there were various histologic components, such as intraneural, Baker¡¯s and vascular cyst.
